 We are available after 12pm to schedule an appointment except on Wednesday the  office is closed. If you need to walk in please call us ahead of time so that we can  fit you into Our schedule.

On Your Appointment Date

 1. Bring a valid insurance card and co-payment (if applicable).
 2. If your insurance card is not yet available we need the following: Insurance  company name, claim address, co-payment , policyholder information. And any  information regarding insurance status prior to services.
 3. Bring in your child immunization record.

 There is always a physician on call at the following number (312) 770-2000 and ask  for In-House Pediatrician. Please call us if you have any concerns regarding your  child’s health. Many times your concerns can be resolved with a return call from the  physician.

Well Exams

 We ask if your child need a physical exam for school, camp, or sports that you that  you schedule an appointment early. Our busiest time of the year is July-August.  There may be a longer waiting period if you wait until the last minute. We will do all  that is possible to accommodate you on short notice.

 The office has a 48-hour turn around time to fill out any forms. If it possible the forms    may be filled out on the day of your visit if time allows.

No Shows and Late appointments

 If you’re unable to keep your appointment please call us as soon as possible, so we may  reschedule your appointment. Know you give the office to offer your appointment slot to  a patient that may need to be seen. If you are more than 15 minutes late we may ask  that you reschedule your appointment for the next available date.

On Call/ After Hours

 Our pediatric staff is on call 24 hours a day. Should you encounter a life threatening  emergency situation we ask that you call 911 or go to the nearest Emergency Room. If  you need to speak to physician the number is (312) 770-2000 ask for the In-House  Pediatrician.

Payment Policy

 If you’re a self-paying patient payment is expected the day services are rendered, unless  other arrangements were made prior to the appointment. All co-payments are expected  on the day of service. If you have an outstanding balance payment is expected on your  next appointment. We accept all major credit cards.
